1. Acknowledgment of Participation

I voluntarily choose to participate in BLW Class/Workshop/Coaching Session offered by BLW Brevard. I understand that baby led weaning involves allowing infants to self-feed solid foods, which carries potential risks.

2. Assumption of Risks

Baby led weaning may pose risks such as choking, allergic reactions, and will likely be messy. My child's safety during BLW is my sole responsibility, and I will closely supervise them at all times. - I have consulted my child’s pediatrician to ensure my child is developmentally ready to start solids.

3. Release of Liability

I hereby release, waive, and discharge BLW Brevard from any and all claims, liabilities, or damages arising from participation in this BLW activity. This includes but is not limited to:

- Injury, illness, or allergic reactions.

- Any food-related incidents occurring during or after the session.

4. Indemnification

I agree to hold BLW Brevard harmless from any claims, legal fees, or added expenses resulting from participation.

5. Medical Emergency

In case of a medical emergency, I assume full responsibility for seeking medical care for my child. BLW Brevard is not responsible for providing medical treatment.

I give BLW Brevard permission to take pictures during the workshop or consultation.
